Summary:

This unique position's primary function is to fulfill Catholic Charities commitment to serve individuals and families in an inclusive and holistic manner. An integral part of the Case Management team, this position will be based in Westchester. The position provides direct assistance for those in need through comprehensive case management services.


Salary: S32.97 per hour, this is a non-exempt hourly position based on a 35 hour work week or $60,005.40 annually.

Location: 204 Hawthorne Avenue, Yonkers, NY, 10705.

Essential Duties and Responsibilities :

• Provides advanced case management to individuals and families, addressing key social determinants of health such as housing, food, income, and healthcare access. Provide effective engagement, assessment, service planning , referral and follow-up using a client-centered approach.
• Provides crisis intervention in emergencies around shelter, food, clothing, and other basic needs through the provision of financial assistance, goods, advocacy, benefits access and access to relevant resources.
• Consistently conducts thorough assessments and develops a service plan that addresses immediate needs and longer-term goals.
• Develops knowledge about and maintains collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
• Develops a capacity to provide or ensure access to a wide range of social service benefits and helps develop and maintain a network of high-quality resources.
• Maintain accurate case notes and work across platforms to meet program deliverables to support reporting to funders and other stakeholders.
• Track authorization
• Actively participates in weekly supervision.

• Attends and participates in monthly d epartment meetings, community meetings, trainings, and other agency-related activities.
• Fulfills operational and other duties as assigned.

Position Type and Expected Hours of Work:

This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs. There may be periods when work hours vary.

Qualifications:

Education and/or experience required:

• Bachelor's degree required in the social services field with at lea st 3-5 years of experience in case management and social services .
• Bilingual English/Spanish required.

Skills, Licenses, and/or competencies required:

Excellent interpersonal skills; the ability to work well independently and as part of a team.

• Knowledge of social service delivery systems and local resources.
• Excellent organizational, time-management and communication skills.
• Knowledge of Unite Us platform is a plus .
