Hospice Social Worker

Heritage Hospice is recruiting a FULL-TIME Medical Social Worker to add to our team! The Medical Social Worker will assist our team on as needed basis helping support our home care patients. Reasons to work for Horizons Home Care: Schedule: Flexible opportunity for individuals with other obligations Opportunity: Grow within the Holiday Health Care network Stability: Family and owned and operated company for 50+ years Job Duties: Assist the physician and other team members in understanding the significant social and emotional factors related to the patient’s health problems, perform evaluations, and participate in the development of the plan of care for the patient Assess psychosocial situation of patient/caregivers and help them understand and follow the medical recommendations made by the care team Provide information and referral services for patients and families regarding practical and environmental needs, including financial assistance programs and resources, and serve as the liaison between patients/families and community agencies Assist the patient and family with personal and environmental difficulties which pre-dispose toward illness or interfere with obtaining maximum benefits from medical care Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders Holiday Health Care is a diverse health services company offering a continuum of care to multiple populations in the Tri-State area.
