Responsibilities
- Provide PTA-level care under supervision to patients in outpatient ortho rehab, swing bed, and related settings.
- Implement and progress treatment plans including therapeutic exercises, ROM, strength, balance, and gait training.
- Monitor patient responses to therapy, document progress, and communicate changes to the supervising PT.
- Assist with mobility, transfers, activity tolerance, and safety education to patients and families.
- Maintain accurate, timely documentation of treatments and outcomes in patient records.
- Collaborate with the interdisciplinary team to coordinate patient care and discharge planning.

Requirements
- Minimum 2 years of Physical Therapy Assistant experience
- Experience in outpatient orthopedics, stroke rehab, and pediatric sports medicine is a plus
- Ability to work the specified Mon-Fri schedule in a LTAC/outpatient swing bed environment
- Strong documentation and communication skills
- Capable of maintaining professional standards in a skilled nursing facility setting
