Hospice RN Case Manager - Full-Time

Location: Serving patients within a 40-mile radius of Plano, TX

Salary: $80,000 - $93,000 annually

Schedule: Monday - Friday | No mandatory on-call

We're looking for dedicated RN Case Managers who feel called to serve, educate, and walk alongside patients and families during their most sacred journeys.

Position Overview:

As a Hospice RN Case Manager, you will oversee a caseload of 12-15 patients, providing skilled nursing, education, emotional support, and coordination of care. You'll collaborate with a team that values compassion, excellence, and faith, while enjoying a manageable caseload and a supportive work culture.

What You'll Do:

• Provide in-home skilled nursing visits for assigned hospice patients
• Coordinate and communicate care plans with interdisciplinary team
• Offer education and emotional support to patients and families
• Document using Axxess EMR
• Participate in a 5-6 PM end-of-day triage huddle to support the on-call nurse as needed
• Uphold and represent Christian values and patient dignity throughout care

Qualifications:

• Current RN license in the state of Texas
• Minimum 1 year of hospice or related nursing experience preferred
• Axxess EMR experience highly preferred
• Passion for end-of-life care and patient education
• Strong communication and critical thinking skills
• A heart aligned with faith-based service and integrity
