Job Summary
We are seeking a detail-oriented and professional individual to join our client’s team as Patient Referral Coordinator. You will be responsible for coordinating patient referrals, scheduling appointments, verifying insurance and demographic information, and maintaining accurate documentation in Epic EMR. This role works closely with patients, providers, and clinical staff to ensure timely access to care while delivering excellent customer service and maintaining HIPAA compliance.
Job Description
Patient Referral Coordination
- Coordinate and expedite appointments for patients referred to by community-based healthcare providers.
- Provide compassionate, patient-centered service by contacting patients to:
- Confirm appointment preferences and availability.
- Verify demographic and insurance information to determine eligibility.
- Guide patients through the scheduling process and answer questions.
Referral Review and Scheduling
- Review referrals for completeness prior to scheduling appointments by ensuring:
- The receiving clinic is accepting community referrals.
- All required referral information and support clinical documentation have been received.
- Patient pre-registration is completed accurately and thoroughly.
Communication and Documentation
- Collaborate with clinic service coordinators to schedule patient appointments in a timely manner.
- Maintain accurate, detailed, and timely documentation of all patient, provider, and staff communications within Epic referrals.
- Transcribe manual (e-FAX) referrals into Epic and create new patient records as necessary.
- Prepare and send appointment confirmation letters with visit details, location information, and required financial and clinical documentation.
- Notify referring providers through Epic Care Link In-Basket when patients cannot be reached and request updated contact information.
- Communicate effectively with internal departments and external providers through Epic In-Basket to facilitate continuity of care and address patient needs.
Process Management and Quality Assurance
- Protect the confidentiality, privacy, and integrity of all patient records in compliance with HIPAA and organizational policies.
- Monitor referral queues to ensure timely processing, minimize delays, and prevent referral backlogs.
- Respond professionally to incoming referral calls while providing exceptional customer service and patient-centered support.

Skills Required
- Experience verifying insurance and scheduling appointments required
- Bilingual Spanish (basic proficiency at minimum) is must.
- Minimum 1 year of relevant experience required
- Prior medical or healthcare setting experience is preferred
Education/Training/Certifications
- High School Diploma/ GED
Additional Requirements
- Shift: 9:00 AM-5:00 PM
Pay Rate
- $22–$26 per hour, based on experience, skills, and market factors
