Registered Nurse Case Manager

RN Case ManagerThis position offers the opportunity to get to know patients and become an integral part of their care team. You will work with broad independence while being part of a cohesive patient-care team. A work/life balance that can match the flexibility needed is provided. The role involves screening, evaluating, re-evaluating, and treating patients. You will coordinate and communicate treatment activities with interdisciplinary team members and patients. Care plans are developed in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission. Periodic reviews of the patient care plan and recertification will be conducted as appropriate. You will coordinate the patient's care plan through interdisciplinary communication and case conferences. Advising other disciplines of discharges in a timely manner is a key responsibility. Ongoing assessment of the patient's clinical, functional, and emotional status will be conducted. Significant changes should be communicated to the patient's physician and colleagues. Documentation of evaluations, treatment goals, and objectives is required. Teaching self-care techniques to patients and caregivers will be part of the role.
