Associate Patient Care Coordinator

Monday–Friday | 8:30 AM–5:00 PM Buda, TX | In-Office Position $25–$27 per hour Full-Time | Home Health Are you an LVN who loves helping patients but is ready to take your nursing experience into an office setting? We’re looking for a caring, organized, and dependable Licensed Vocational Nurse (LVN) to join our team as a Patient Care Coordinator (PCC). WHAT YOU’LL DO Triage patient phone calls under the direction of the RN Coordinate care between patients, practitioners, field staff, and the office Assist with scheduling and coordinating PRN visits when needed Obtain and transcribe practitioner orders into the EMR Receive and process external orders Provide patient and caregiver education under RN direction Assist with patient discharge coordination Assist with referrals, chart reviews, and required patient documentation Communicate with referral partners, practitioners, patients, and families Follow infection-control, HIPAA, Medicare, Medicaid, and agency requirements Participate in QAPI, in-services, and professional development Assist with patient visits when needed and perform other duties as assigned WHAT WE’RE LOOKING FOR This position requires an active LVN license. Valid/current LVN License CPR or BLS Certification Valid Driver’s License Valid Auto Insurance Reliable transportation Strong communication skills Excellent organization and time-management skills Ability to work as part of a team Professional, dependable, and patient-focused attitude Ability to work under the direction of an RN Healthcare experience required; home health experience is a plus!
