LPN Home Health

Job Summary We are seeking a Registered Nurse or Licensed Practical Nurse to join our growing team at Jethro Health System. For this role, you will provide high-quality nursing care to patient in their home, ensuring their needs are met with compassion and professionalism. Home Care LPN Responsibilities Assess the patient's condition during every visit and document your observations Perform different tasks including vital signs and medication review Administer medication as prescribed by the patient's Physicians Perform wound care and assess the progress of healing Educate patients and their families on proper home health care strategies and procedures Coordinate with Physicians and anyone else involved with the patient's care plan Make recommendations for devices/tools that might improve the patient's quality of life Listen to the patient and respond to concerns or requests. Complete all clinical documentation accurately and timely Qualifications Active Licensed Practical Nurse (LPN) license in Massachusetts 1-2 years of clinical experience (home care experience preferred) Strong communication and organizational skills Ability to work independently in the field Familiarity with EMR systems Reliable transportation Schedule & Benefits Flexible scheduling Competitive hourly rate (based on experience) Supportive and collaborative team environment Opportunity to make a meaningful impact in clients' lives Why Join Us?
