Advanced Practice Provider

HarmonyCares seeks an Advanced Practice Provider to deliver home-based primary and palliative care to medically complex, home-limited patients. You will conduct in-home and telehealth visits, manage chronic and acute conditions, coordinate with an interdisciplinary team, and develop personalized care plans to prevent avoidable hospitalizations. In this mission-driven role, you’ll collaborate closely with physicians, nurses, and care managers to improve access, outcomes, and patient experience while practicing in a supportive, patient-first culture with opportunities for growth.
