Cardiac Navigator RN

Piedmont Healthcare is seeking a Cardiac Navigator RN to guide patients through the continuum of cardiac care. This role coordinates care from diagnosis through follow-up, ensuring timely appointments, test scheduling, and smooth transitions between inpatient, outpatient, and rehab services. The navigator provides evidence-based cardiac education, medication and lifestyle counseling, and post-discharge follow-up to reduce readmissions. Collaborating closely with cardiologists, nurses, and allied health professionals, the RN supports care plans, tracks outcomes, and advocates for patients and families in a collaborative, patient-centered environment.
