RN Case Manager - Greenwich, CT (BONUS!)

Constellation Home Care is looking for a compassionate and skilled Registered Nurse (RN) Case Manager to join our team! This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities.Constellation Health Services was founded to be an innovative leader in the post-acute continuums by the development of core teams that embody clinical excellence, uniqueness within their communities and act with authentic intention. We are dedicated to providing the highest quality of patient care with compassion and respect for each person.Responsibilities:Provide skilled nursing and head-to-toe assessments to the adult and geriatric population.
