Wage: $22.50-$25.00/hr + shift differentials DOE
Differentials: $2 PM | $3 NOC
Status: Full-time or Part-time
Sign-On Bonus:
• $3,000 Full-time NOC CNA ONLY
• $1,500 Part-time AM, PM or NOC CNA
Shift Options:
AM: 6:00 AM–2:30 PM
PM: 2:15pm-10:45pm
NOC: 10:30 PM–6:30 AM
(Includes every other weekend and holiday)

Job Summary:
- NA/R is a helpful, caring, compassionate, positive, patient, hardworking, resilient, caregiver, co-worker, team player, hairdresser, liaison, and a friendly face for our residents.
- The NA/R is responsible for assisting the residents with Activities of Daily Living (ADL’s) through daily nursing care as described in each resident’s plan of care.
- The NA/R carries out simple treatment procedures, reports to and follows the instructions of the charge nurse.
- The NA/R works closely with the activities department, volunteers, housekeeping, and all other ancillary departments.
- NA/R’s frequently have contact with the resident’s family members.
Qualifications
Position Requirements:
- The state of Minnesota requires all nursing assistants/registered to have completed a Minnesota approved course with completion of the final test for certification. The nursing assistant must be on the Minnesota Board of Nursing Registry upon hire or completion of class and test within 4 months of hire.
- The NA/R must have at least a minimum of 12 hours of in-service education as set forth by the Minnesota Board of Nursing. A minimum of 12 hours annual of education will be provided by GVCC.
- Must attend at least 75% of all monthly nursing assistant meetings.
- Ability to communicate with other staff members and department managers.
- Must be able to read, write, speak, and understand the English language.
- The NA/R must have the ability to communicate with others with a variety of differing values, attitudes, capabilities, and dispositions, while demonstrating a friendly and cooperative attitude.
- Must be able to work effectively as a team and work independently.
- Must be able to relate information concerning a resident’s condition.
- Must be able to cope with mental and emotional stress of the position.
- Must be able to function independently, professionally, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
- The must be able to meet the following physical requirements to complete duties listed in the job description:
- Must be able to bend, stoop, squat, crouch, and kneel occasionally.
- Must be able to twist, reach above shoulder height, push/pull wheelchair or cart frequently.
- Must be able to be on their feet for most of their shift as assigned.
- Must be able to push, pull, move, or lift a minimum of 50 pounds to a minimum height of 4 feet and be able to push, pull, move or carry such weight a minimum distance of 50 feet.
- Must be able to lift 50-200 pounds occasionally.
- Must be able to read and understand handwriting, regular print and directions. Must be able to write in a legible manner. Vision must be appropriate to read regular 10-point print with or without corrective lens.
- Must be able to hear sounds of resident calls for assistance when the resident is not within their sight.
- Must be able to work independently up to 100% of the time.
- Must be able to work every other weekend and holiday.
- Must complete Criminal Background check as required to assure qualification for employment.
- Must understand and practice Standard Precautions.
Job Responsibilities:
- Assist with Activities of Daily Living – Perform hands on care for all ADL’s as contained in the resident’s plan of care. This includes: dressing, grooming, bathing, oral cares, toileting, restorative care, and ambulation and position changes as written in plan of care.
- Provide Dietary assistance to the residents-assist with eating, drinking, monitoring intake according to plan of care. Offer fluids/nourishments as needed between meals and as necessary. Encourage participation in set up snack time.
- Bladder and Bowel Care- Assist residents with toileting as written in their plan of care. This may include, transferring, peri care, colostomy/catheter emptying or cleansing, and reporting outputs and change to the charge nurse as necessary.
- Monitoring, observations, and vital recording – the NA/R takes resident’s vital signs including blood pressure, temperature, pulse, respiration, and oximeter readings. Observe abnormal changes in body functions and or behavior (eating habits, skin conditions, mental status, mobility, etc.) and alert the charge nurse to changes.
- Read and understand the resident plan of care - The NA/R can read and understand the plan of care for each resident they care for. Follow instructions as contained in the plan of care. NA/R must know and understand where to find the information on each resident at each station. Review the plan of care with the charge and alert the RN case manager to changes with the residents.
- Offer and provide support and encouragement to residents and/or family-Provide needed assistance and encouragement in getting residents to and from activities that provide opportunities for socialization with others. Promote residents’ right to be free from abuse, mistreatment, and neglect and report to appropriate facility staff any instance of such treatment. Offer support and comfort to residents who are dying and his/her family. Include residents in on conversation with cares. Inform residents of what you are doing with them to promote participation.
- Continuous Improvement – NA/R actively participates in and implements quality performance in all aspects of job responsibilities and functions. This includes interactions with resident staff and visitors. Participate in and promote teamwork. The NA/R recommends, seeks and/or implements new methods and means of providing quality service and continuous improvements.
- Ensure that care is provided in accordance with the residents’ wishes.
- Participate in facility surveys made by authorized government agencies as requested by the Director of Nursing.
- Attend various committee meetings of the facility as required.
- Understand and perform job functions in relationship to environmental care.
- Maintain a clean environment for the residents. Follow proper infection control practices in all aspects of work.
- Recognize safety responsibilities of employee for the resident. Report occupational hazards and/or incidents of injury to appropriate personnel, in accordance with facility policy and procedures.
- Recognize employee’s responsibility for his / her own safety while on the job and for the safety of others. Recognize and remove environmental dangers.
- Complete maintenance service request slips for equipment in need of repairs. Include date, time and signature on your request.
- Apply principles of good body mechanics when lifting and moving objects.
- Use transfer techniques consistent with proper body mechanics.
- Participate in disaster planning drills (fire, tornado, severe weather, etc.) when on duty.
- Maintain calm manner in tense situations.
- Maintain awareness and adhere to Use safety devices, when appropriate, and implement their use.
- Develop knowledge of actions to be implemented in emergency situations.
- Practice fire safety and accident prevention procedures.
- Follow policy and procedures governing the operation of Glenwood Village Care Center.
- Attend orientation and receive instruction regarding department duties.
- Attend general orientation and assigned in service education programs.
- Understand use of department procedure manual.
- Must understand and adhere to Vulnerable Adult Abuse Prevention Policy.
- Must adhere to HIPPA privacy rules and regulations.
- Must always uphold resident rights
- Demonstrate dependable professional work habits. Adhere to requirements set forth in personnel policy manual.
- Report absences, according to policy.
- Come to work on time and when scheduled.
- Make suggestions, complaints, or grievances in a constructive manner.
- Handle work-related situations regarding staff with confidentiality.
- Maintain good personal hygiene and grooming. Come to work clean and neat.
- Wear the proper uniform. Jeans may be worn on Fridays per personnel policy manual. No open toes shoes are allowed for direct care givers. Proper uniform for the nursing department is scrub uniforms.
