In-Home Nurse Practitioner — Medicare Wellness & Risk-Adjustment (Flexible Schedule)

A healthcare solutions provider is looking for a highly skilled Nurse Practitioner to conduct in-home wellness risk adjustment assessments for the Medicare population. The NP will perform annual wellness visits and assist in closing quality care gaps while collaborating with teams to enhance care delivery. Candidates should have a valid NP License, at least 3 years of patient care experience, and prior risk assessment experience is a plus.
