- Conduct in-person home visits to assess patient safety, monitor changes in health, and provide clinical coaching-including medication reviews.
- Implement and monitor individualized care plans, offering guidance and education via our digital care app.
- Coordinate appropriate referrals to community and clinical resources.
- Establish strong, trust-based relationships with patients and caregivers to promote engagement and reduce caregiver fatigue.
- Document assessments, interventions, and patient outcomes in accordance with regulatory and agency requirements.
- Participate in team meetings and contribute to projects and initiatives as needed.
- Active Registered Nurse (RN) License in the state of Massachusetts.
- Minimum 2 years' recent experience providing direct care to elders or adults with disabilities.
- Valid driver's license, reliable vehicle, and auto insurance.
- Experience with case management , care planning , and clinical assessments , ideally in eldercare or chronic disease management.
- Strong computer literacy, including Microsoft Office and case management systems.
- Bilingual proficiency is a plus and may lead to enhanced compensation.
