Summary:

The Registered Nurse (RN), Support & Assessment Nurse collaborates closely with the Director of Nursing Services to conduct resident assessments, develop individualized care plans, and coordinate nursing services within the community. This role provides direct nursing support, ongoing clinical assessments, and care coordination while ensuring residents receive high-quality, person-centered care in accordance with physician orders and all applicable federal, state, and local regulations.

Duties and Responsibilities:

• Conduct initial resident evaluations and ongoing nursing reassessments by reviewing medical records, gathering health information, assessing functional abilities, and determining appropriate levels of care.
• Develop, implement, and update individualized resident care plans in collaboration with physicians, department leaders, and interdisciplinary team members.
• Complete required assessments, reassessments, transfers, discharges, and documentation within established timeframes.
• Perform skin assessments and support preventative skin and wound care initiatives, including monitoring skin integrity and implementing early intervention measures.
• Provide direct nursing services and document resident visits, progress notes, care plan updates, and clinical observations accurately and timely.
• Process physician orders and communicate changes in resident condition or care needs to physicians, families, and appropriate team members.
• Coordinate services with outside providers including pharmacy, hospice, home health, and rehabilitation agencies to ensure continuity of care.
• Meet with residents and families to address questions, concerns, and care-related matters while promoting positive customer relations.
• Participate in Health Maintenance Meetings and interdisciplinary care conferences to review resident service plans and coordinate care updates.
• Monitor quality-of-care indicators, review medical records, and assist in implementing quality improvement initiatives and regulatory compliance standards.
• Participate in ongoing education, workshops, seminars, and professional development activities as required.
• Serve as a representative of the community and nursing department through professional interactions with families, referral partners, and healthcare providers.
• Report concerns related to resident safety, abuse, neglect, or mistreatment in accordance with company policy and regulatory requirements.
• Perform additional duties as assigned by the Director of Nursing Services.

Knowledge, Skills, and Qualifications:

• Current Registered Nurse (RN) license in good standing within the state of practice required.
• Minimum of 2–3 years of nursing experience, preferably in senior living, assisted living, long-term care, or related healthcare setting.
• Bachelor of Science in Nursing (BSN) preferred.
• Strong understanding of federal, state, and local healthcare regulations applicable to senior living communities.
• Excellent clinical assessment, critical thinking, problem-solving, and communication skills.
• Ability to manage multiple priorities in a fast-paced environment while maintaining professionalism and high standards of care.
• Proficiency in clinical documentation, report writing, and electronic systems.
• Strong knowledge of geriatric care practices, chronic disease management, and resident-centered care models.
• Ability to read, write, and communicate clearly in English.
• Must complete required TB screening and successfully pass all applicable background clearances prior to employment.
• Ability to work flexible hours, including weekends, evenings, and holidays as needed.

Mental and Physical Requirements:

• Ability to stand, walk, bend, and move throughout the community for extended periods of time.
• Ability to lift, move, or assist with equipment or supplies up to 25–50 pounds occasionally.
• Ability to remain calm, professional, and compassionate in stressful situations.
• Ability to assist during resident emergencies or evacuations when necessary.
