Case Management Nurse

Job Title: RN Case Manager / RN-Supervisor Locations: Washington State/ California/ Alaska (with housing)/Wisconsin/ Colorado/ Arizona Shift: Day and Night available 36 and 40hrs Guaranteed WEEKLY PAY: $2500-$3000 (depends on location) Job Summary: We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to coordinate patient care and ensure effective discharge planning. The ideal candidate will have a strong background in acute care settings and proven experience in care coordination. Key Responsibilities: Coordinate and manage patient care plans across the continuum of care Perform comprehensive patient assessments in an acute care setting Develop and implement discharge plans to ensure safe and timely transitions Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes Facilitate care coordination, including referrals and follow-ups Monitor patient progress and adjust care plans as needed Ensure compliance with hospital policies, regulatory standards, and documentation requirements Educate patients and families on post-discharge care and available resources Requirements: Active RN license (state-specific or compact) Minimum experience in acute care setting Proven experience in discharge planning and care coordination Strong communication and organizational skills Ability to work collaboratively in a fast-paced environment
