Case Worker

Head Start Case Worker – GLEAMNS Human Resources Commission, Inc. We provide quality services to families, focusing on education, employment, and self‑sufficiency. Join our community‑based team to help children thrive. Responsibilities Provide social services to improve children’s and families’ social and psychological functioning. Support health, parenting and academic needs of children and families within the Head Start Program. Interview clients individually, in families or groups to assess needs and determine required services. Develop coordinated family service plans, including healthcare and other supports. Initiate contacts with community support services to address comprehensive needs. Maintain case history records and prepare monthly reports on attendance, medical/dental screenings, follow‑ups, and immunizations. Refer clients to community resources (job placement, debt counseling, legal aid, housing, medical treatment, financial assistance) and provide application guidance. Serve as liaison among students, households, schools, child guidance clinics, courts, protective services, doctors, and other contacts. Counsel students with behavioral, school progress, or mental/physical impairment issues. Organize family meetings, provide training, take minutes, and arrange follow‑up home visits. Evaluate individual characteristics to determine needs or eligibility. Collaborate with other professionals to assess client needs or plan treatments. Prepare client file documentation and periodic reports for the Family & Community Partnerships Manager. Perform other social services as required. Qualifications B.S. Degree in Social Services or related field, or equivalent education and experience. Minimum of two years of work experience in a social services environment. Proficiency with Microsoft products and other relevant computer applications. Strong teamwork and communication skills. Valid South Carolina Driver’s License required; a Commercial Driver’s License must be obtained within 90 days of employment. Ability to meet physical requirements: prolonged periods of sitting at a desk, ability to lift up to 15 pounds. Benefits South Carolina State Health, dental, and vision insurance.
