Responsibilities include:/pulliWork with the Care Right There team to implement client-specific goals developed by nursing and care management/liliTrack progress and adjust approaches as goals are met, modified, or changed due to improvement or decline in condition/liliDocument goal progress clearly and consistently/liliFacilitate daily cognitive stimulation activities such as memory games, orientation exercises, puzzles, reading, and conversation engagement/liliLead functional activities to support independence (dressing practice, fine motor tasks, handwriting, sorting, etc.)/liliAssist with physical therapyaligned exercises as directed by the RN or therapist (range of motion, mobility exercises, balance support, safe ambulation)/liliEncourage meaningful, structured activities (music therapy, art, cooking, routine-based tasks, and personalized engagement)/liliAssist with bathing, grooming, dressing, toileting, feeding, and hygiene/liliSupport safe mobility, transfers, and prescribed exercise programs/liliMaintain safety through supervision, fall prevention, and behavioral redirection/liliUse appropriate dementia-care techniques, including validation, redirection, calm communication, and routine-building/liliManage and document behavioral changes, confusion, anxiety, wandering, or agitation/liliMaintain structured daily routines that promote stability and reduce cognitive stress/liliUse caregiver technology tools (apps, tablets, AxisCare, communication tools) to:/liulliLog care notes/liliTrack goal progress/liliFollow digital care plans/liliCommunicate with the office and nursing team/li/ulliUse simple tech with clients when appropriate (music playlists, brain-game apps, memory prompts, digital photo albums)/liliPrepare nutritious meals and support hydration/liliPerform light housekeeping related to client care (laundry, dishes, room upkeep)/liliMaintain a clean, organized, and therapeutic environment/liliAccompany clients to appointments when needed/li/ulpRequirements:/pulliActive NJ CHHA certification (preferred)/liliDementia/Alzheimers training or certification/liliMinimum 1 year of memory care experience/liliAbility to commit to live-in care schedules where you will reside at the patients home 24/7, 7 days a week/liliReliable transportation/liliStrong communication, patience, and problem-solving skills/li/ulpCompensation: $250.00 - $300.00 per day/ppCare Right There Home Care/ppTheres no place like home Just as growing older can allow for a more relaxed lifestyle, it can also present new obstacles and challenges.
