Position Overview The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on the assessment, the therapist works to develop a treatment plan, performs interventions to improve and enhance patient well-being, and evaluates progress. Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care. Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician. Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice. Instruct and inform patients, families/caregivers, and the healthcare team about the plan of care, functional limitations, and progress toward goals. Must Haves Current PT license in the state where services are rendered. Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Ability to reliably commute to multiple patient residences daily within the geographical area assigned.
