Youth Case Manager - College & Career Readiness Mentor

The YMCA of Greater New York is seeking a Case Manager to support college and career readiness. You will mentor students, conduct intake, and develop individualized plans to help youth achieve academic goals. The role involves leading groups, coordinating internships, and maintaining accurate case notes across DYCD Connect and YMCA databases.
