Physical Therapist (PT) - Home Health | Northeast Westchester County | PRN & Part-Time | $65 - $85 Per Visit.

Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients in their homes in and around Rye Brook, Mamaroneck, Port Chester, Scarsdale, Purchase, and Harrison, NY.

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.

Treat one patient at a time, manage your availability, and provide individualized care with support from an established clinical and administrative team.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Rye Brook area.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety, and overall function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.
• Build and manage your caseload based on your availability and preferred coverage area.

Compensation:

Part-Time W-2:

• $65 per follow-up visit.
• $75 per 4-unit evaluation.

PRN:

• $72 per 4-unit follow-up visit.
• $85 per evaluation.

Qualifications:

• A n active Physical Therapist license in New York is required.
• Reliable transportation required.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.
• Home health experience is preferred but not required.
