Hospice Registered Nurse

SHC WVUHS Home Care is seeking a Registered Nurse Case Manager to deliver comprehensive home health nursing care and to coordinate patient care with families and the interdisciplinary team. The role requires guiding the patient through treatment plans, assessments, and ongoing education in a home setting. The ideal candidate will have an active RN license, CPR certification, and a valid driver’s license.
