Registered Nurse (RN) Case Manager – Hospice

LANCASTER, Pennsylvania

• Coordinate and manage hospice care for your assigned patients
• Perform comprehensive patient assessments and develop individualized care plans
• Collaborate with physicians and the interdisciplinary hospice team
• Provide compassionate education, guidance, and support to patients and families
• Maintain accurate and timely documentation in accordance with hospice regulations
• Build meaningful relationships with patients and families during an important time in their lives

✔️ Active Pennsylvania RN license
✔️ Hospice or home health experience preferred
✔️ Excellent communication and organizational skills
✔️ Compassionate, patient-centered approach
✔️ Valid driver’s license and reliable transportation
