Job Summary:
We are seeking an Admissions Coordinator to join our team at River Bend Health and Rehabilitation. The Admissions Coordinator is responsible for ensuring a seamless and efficient admission process for new residents, while maintaining a high level of customer satisfaction. Responsibilities:

• Coordinate and facilitate the admission process for new residents, including scheduling tours, conducting assessments, and gathering necessary documentation

• Develop and maintain relationships with referral sources, including hospitals, rehabilitation centers, and other healthcare providers

• Ensure accurate and timely completion of all admission paperwork and documentation

• Communicate effectively with residents, families, and healthcare professionals to ensure a smooth transition to our community

• Participate in care planning meetings and contribute to the development of individualized care plans

• Maintain accurate records and reports, including census data and admission statistics
