This position provides health education and patient counseling to control and prevent disease and promote optimum health and wellbeing of clients. Models Eskenazi Health values of Professionalism, Respect, Innovation, Development and Excellence/liliPerforms nursing services as a health team member in Outreach clinics/liliProvides health education and patient counseling to control and prevent disease and promote optimum health and wellbeing of clients/liliResponds to health needs of patients when seen in the clinic by identifying and assessing health care requirement of problems expressed/liliAssists physicians and other team members in clinics/liliDevelops and implements nursing care interventions based upon goals in the Plan of Care/liliRefers clients to appropriate agencies/organizations to promote continuity of care for patients and families based upon nursing assessment responses from the client/family/liliWorks collaboratively with physicians and other health professionals to implement treatments, dispense medications, and give medication and injections in accordance with prescribed orders, instructions/liliWorks as a member of a health team to counsel clients and their families in health education, medication education and treatment instructions/liliAttends staff meetings regularly and punctually to share information regarding patients, policies/procedures, peer review, and resource material/liliCompletes and submits internal nursing audits regularly/liliInterviews patients and records health history, prepares, maintains and uses records for reports for direct client services/liliAlters activities/behaviors to reflect/ensure adequate care appropriate to the age of patients served (i.e. adolescent, geriatric, pediatric, neonatal, adult) as directed in the area/unit age-specific and/or job specific competencies/liliEnsures the timely completion of all documentation per policy/liliMeets productivity goal as set forth by the agency/liliDemonstrates an understanding of The Recovery Model/li/ulpJob Requirements:/pulliMinimum of twelve months of clinical experience in Behavioral Health/liliMinimum of 12 months clinical nursing experience as a Registered Nurse/liliCurrent Indiana License as Registered Nurse/liliMust have valid Indiana Drivers License/li/ulpKnowledge, Skills Abilities:/pulliSkills in nursing, health assessment, teaching/counseling, effective interpersonal communications/liliKnowledge of: disease processes and their treatment/prevention, basic science, current clinical protocol/nursing techniques, medication/drugs and their use/side effects, community resources referrals, culture attitudes/custom appropriate departmental procedures and local/State/Federal laws/regulations/liliKnowledge and skills necessary to provide care appropriate to the age of patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess date/interpret appropriate information needed to identify each patients requirements relative to age-specific needs/liliAbility to use appropriate communication skills.
