Role Overview This position serves as a clinical pharmacist caring for adult patients on hospitalist and cardiology services. The pharmacist works closely with physicians, advanced practice providers, nurses, care managers, and other members of the interdisciplinary healthcare team to optimize medication therapy and improve patient outcomes across a broad range of acute and chronic disease states. Responsibilities include supporting patients throughout their hospitalization, addressing medication‑related barriers to care, and facilitating safe transitions between care settings. Key Responsibilities Acute Care/Infusion Review provider orders for correct drug choice, dosage, form, and potential interactions or allergies; contact providers for clarification. Dispense and compound medications, including drug therapy‑related devices, adhering to OhioHealth pharmacy policies. Initiate, monitor, and modify drug therapy in collaboration with providers; provide drug information to the healthcare team and patients. Conduct therapeutic drug monitoring, antimicrobial stewardship, and discharge medication reviews. Counsel patients on medication usage, non‑prescription drug selection, and device use. Precept pharmacy students and residents; develop educational programs for pharmacy, nursing, and medical staff. Direct and guide pharmacy technicians and support personnel. Ambulatory Care/Population Health Manage drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with providers. Develop individualized care plans to optimize therapeutic outcomes, prevent adverse events, and increase adherence. Partner with patients, caregivers, and healthcare professionals to promote disease prevention, wellness, triage, referral, and self‑management. Assist in guideline and policy development, and participate in performance‑improvement initiatives. Precept students and residents; provide educational programs. Direct and guide pharmacy technicians and support personnel. Retail Review provider orders and dispense medications and related devices in accordance with pharmacy standards and policies. Ensure proper labeling, billing procedures, and medication safety in retail settings. Counsel patients on therapies, non‑prescription drug selection, and proper use. Initiate, monitor, and modify prescriptions in collaboration with providers. Precept students and residents; provide educational programs. Direct and guide pharmacy technicians and support personnel. Medication Safety, Stewardship & Utilization Lead safety, stewardship, and utilization strategies for designated sites; facilitate activities of committees and councils. Develop, enforce, and maintain policies that promote safe, effective, and cost‑efficient medication therapy. Use data and best‑practice standards to identify opportunities for proactive improvement in the medication‑use system. Integrate medication improvement initiatives with clinical and operational programs. Ensure compliance with evidence‑based recommendations, national patient‑safety standards, and regulatory requirements. Precept students and residents; provide educational programs. Direct and guide pharmacy technicians and support personnel. Qualifications Bachelor of Science or Doctor of Pharmacy degree from an accredited institution. Current Ohio pharmacist license. Previous pharmacy experience. Basic Life Support certification (current or obtained during orientation). Completion of a Pharmacy Residency (PGY1) or equivalent hospital experience (required); PGY2 or specialty residency preferred. Pharmacy Board certifications or other relevant certifications. Advanced‑level degree in a health care field (optional). Collaborative practice experience. Advanced Cardiac Life Support certification (as determined by manager). Pharmacokinetic drug dosing experience. Drug therapy management skills. Additional Information Schedule : Mix of day and evening shift responsibility with every 4th weekend and 2 major holidays per year. Night shift rotation required to fill planned time off. Progressive Practice Model : Mixed model where all pharmacists rotate through central pharmacy and 1–2 clinical areas. Supported clinical areas include general medicine, infusion/oncology, operating room, cardiac care, critical care, emergency medicine, trauma, anticoagulation clinic, and more.
