Roles & Responsibilities

• Evaluate patients' physical therapy needs and craft personalized treatment plans
• Implement therapeutic programs that help patients regain independence and improve quality of life
• Collaborate with a multidisciplinary team to ensure comprehensive patient care
• Maintain accurate and thorough documentation of patient progress and outcomes
• Empower patients and their families with education and support throughout the rehabilitation process

Qualifications

• Bachelor of Science in Physical Therapy
• Active Physical Therapist licensure in Vermont and New Hampshire, or a compact license
• Minimum of one year of experience in home health physical therapy
• BLS certification required

Education

• BS in Physical Therapy

Knowledge & Skills

• Expertise in physical therapy assessment and treatment
• Knowledge of home health care practices and protocols
• Strong communication skills for patient and team interactions
• Proficient in clinical documentation
• Familiarity with PointClick EMR system
