What You’ll Do As a Home Health Occupational Therapist, you will: Perform comprehensive OT evaluations Develop and update individualized plans of care Assess functional limitations and barriers to independence Address activities of daily living, including bathing, dressing, grooming, toileting, and meal preparation Provide therapeutic activities to improve functional performance Assess home safety and fall risks Recommend adaptive equipment and modifications Provide training in safe transfers and functional mobility Develop home programs based on patient goals Educate patients, families, and caregivers Monitor progress and adjust treatment plans as appropriate Complete reassessments and discharge planning Collaborate with the interdisciplinary home health team Complete accurate and timely documentation using WellSky What We’re Looking For Active Maryland Occupational Therapist License Required Graduate of an accredited Occupational Therapy program Reliable transportation Strong clinical judgment and communication skills Ability to work independently in patients’ homes Excellent organizational and documentation skills Home Health Experience Preferred, but Not Required WellSky Experience is a Plus Therapy That Meets Patients Where Life Happens A clinic can simulate daily activities.
