The Social Worker collaborates with an interdisciplinary team to facilitate care planning, support transitions across the continuum of care, and advocate for patient and family needs. Schedule Details Location: 100% onsite at 5323 Harry Hines Blvd, Dallas, TX. Work Schedule: Four 10‑hour weekday shifts (8:00 a.m. – 6:30 p.m.). No weekends or holidays. Experience and Education Master's Degree in Social Work from an institution certified by the Council of Social Work Education. Applicable state license required; LCSW (Licensed Clinical Social Worker) required upon hire or must be obtained within one year. Preferred Experience delivering direct clinical counseling services; background in palliative care or hospice preferred. Certification in Advanced Hospice and Palliative Care preferred. Job Duties Serve as liaison for transitional care planning between patient, caregivers, and palliative care/clinical teams; assist clients to navigate healthcare systems, identify and link clients with resources. Attend administrative and clinical meetings relevant to operations of the palliative care program; distribute relevant information to departmental staff. Participate in interdisciplinary team meetings, care planning, and ethics consultations. Work with multidisciplinary team to identify and develop dispositions plans. Conduct independent psychosocial assessment and direct clinical counseling; provide counseling and psychotherapy for individuals, couples, and families. Provide grief, loss, and bereavement support. Provide psychosocial education to patients and family caregivers about coping strategies, hospice and palliative care philosophy, and non-pharmacological symptom management. Utilize crisis intervention and problem solving skills to facilitate and enhance patients’ and families’ ability to meet acute and ongoing medical and psychosocial needs. Advocate on behalf of patients and families; facilitate advance care planning. Facilitate and/or support existing psychoeducation support groups. Provide in‑services or education workshops to other service providers and/or organizations to promote the palliative care philosophy and program. Maintain effective documentation of social work activities. Perform other duties as assigned.
