Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities
- Perform clinical assessment to identify patient needs and determine appropriateness for physical therapy.
- Document and review patient care plans, including progress notes and treatment plans, and collaborate with the interdisciplinary team.
- Provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and assessments.
- Instruct patients and families on safe transfer techniques and the use of therapeutic equipment.
- Educate patients and families about therapeutic procedures, considering the patient's specific needs.
- Oversee and assist team members in delivering patient care consistent with individual needs.

Compliance / Notes
- Must read, write, and speak fluent English.
- Applies concepts of infection control and universal precautions in patient care.
