From acute care settings to critical access hospitals and outpatient facilities, you will ensure a safe and seamless continuum of care for patients post-discharge.

Responsibilities:

• Personalized Patient Care : Provide comprehensive evaluation, create individualized treatment plans, and deliver physical therapy services to patients of all ages and diverse backgrounds within the Baton Rouge community.
• Direct Patient Interaction : Work closely with patients and their families, establishing strong therapeutic relationships and ensuring clear communication to facilitate effective treatment outcomes.
• Multidisciplinary Collaboration : Collaborate with a team of healthcare professionals, including physicians, occupational therapists, speech therapists, and nurses, to develop integrated care plans and promote holistic patient wellness.
• Acute Care and Outpatient Services : Offer your expertise in a variety of settings, including acute care facilities, outpatient clinics, and rehabilitation centers, to address a wide range of medical conditions and injuries.
• Documentation and Compliance : Maintain accurate and timely patient records, ensuring adherence to regulatory standards and ethical guidelines.
• Continuous Professional Development : Stay up-to-date with advancements in physical therapy techniques, technologies, and research to continually enhance your skills and provide the best possible care.

Qualifications:

• Louisiana Licensure : Must possess a valid license to practice as a Physical Therapist in the state of Louisiana.
• Education : Graduation from an accredited Physical Therapy program
• Clinical Experience : Previous experience in physical therapy is desirable, but new graduates are welcome to apply as well.
• Compassion and Empathy : Display a genuine commitment to patient care, demonstrating empathy, patience, and understanding in all interactions.
• Communication Skills : Strong verbal and written communication skills to effectively interact with patients, families, and the interdisciplinary healthcare team.
