The Office for Children with SpecialHealth Care Needs (OCSHCN) may be the place for you! OCSHCN is seeking an Early Childhood ServicesCoordinator. OCSHCN provides comprehensive services for children and youth with special health care needsthrough early intervention, pediatric specialty clinics, care coordination, case management, therapies, andcollaborations.

Kentucky Early Intervention System Point of Entry (KIPDA) serves families in Bullitt, Henry, Jefferson,Oldham, Shelby, Spencer, and Trimble counties. The selected candidate would need to provide support tofamilies in these areas on a day-to-day. Some case management duties will be completed virtually, and somewill be completed via home visitation.

Responsibilities and Duties:
  • Assist parents of infants and toddlers with disabilities in obtaining access to needed early intervention services; travel throughout the community visiting families, as well as conducting some meetings via teleintervention for children referred to Kentucky's Early Intervention Program.
  • Facilitate and participate in the development, review and evaluation of Individualized Family Service Plans (IFSP) meetings with families and service providers; coordinate the provision of early intervention services and other services that the child may need.
  • Prepare and maintain confidential case records; monitor's family and child's progress by performing follow up assessments in relation to the treatment plan; performs all duties in compliance with applicable federal and state regulations and laws.
  • Maintain each early intervention caseload in the Technology-assisted Observation and Teaming Support System (TOTS) with accurate data entry in all applicable fields; enter IFSPs and service logs within regulatory timelines.
  • Facilitate the development of a transition plan to preschool, school or other appropriate service; assist families in finding community resources or assign file to the District Child Evaluator Specialist in a timely manner to ensure continued delivery of service.
  • Perform other duties as assigned.

  • Strong verbal and written communication skills, time management and organizational skills are necessary
  • Experience in performance and operations management
  • Proficient in MS Office

Minimum Requirements

EDUCATION: Graduate of a college or university with a bachelor's degree.

EXPERIENCE, TRAINING, OR SKILLS: Must have two years of paid experience working with children and/or families with children ages birth through five years.

Substitute EDUCATION for EXPERIENCE: A master's degree in early childhood education, social work, psychology, human services, or related field will substitute for one year of the required experience.

SPECIAL REQUIREMENTS (AGE, LICENSURE, REGULATION, ETC.): Must possess a valid driver's license prior to employment in this job classification. Must maintain any required licensure(s), certification(s), or other credentials for the length of employment in this job classification. Working Conditions

Incumbents working in this job title perform duties in an office setting. Frequent travel is required.

Probationary Period

This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
