This role involves engaging with high-risk members to develop and implement comprehensive care plans. You'll be collaborating with a multidisciplinary team to ensure the best outcomes for patients across multiple counties including Escambia, Santa Rosa, Okaloosa, and Walton.

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for care plan implementation
• Reassess cost-efficient, appropriate levels of care
• Coordinate non-medical support to ensure treatment compliance
• Work with physicians, social workers, and claims professionals
• Engage specialty resources for optimal patient outcomes
• Maintain detailed records of clinical and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate by protecting privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Florida License required, will accept compact license
• Associates or Bachelor Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Proficiency in Home Care Home Base/PointCare and OASIS collection
• Experience with OASIS/HCHB, wound care, PICC/IV care, lab collection, drain care, and post-surgical care is helpful
