As one of the community's industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our eight-county territory. Transitions LifeCare's goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.

• Wake
• Durham
• Johnston
• Franklin
• Harnett
• Orange
• Chatham

The schedule for this position is Thursday-Monday 7:00am-3:30pm; rotating holidays.

Responsibilities:

• Provide high-quality and compassionate care to patients per their individualized care plan
• Works collaboratively with the interdisciplinary team (IDG) in achieving patient care goals
• Maintain comfort, privacy, and dignity of our patients
• Promptly reporting pertinent information to the immediate supervisor

Requirements:

• Current CNA listing with NC Home Health Aide Registry
• Must have reliable transportation
• Hospice or home health experience preferred
• Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
