Job DescriptionPrimary Care Physician—St. Louis, MO\nCOMPENSATION: $260,000 BASE TOTAL POTENTIAL $312,000 + Full Benefits\nWe’re a new, innovative healthcare provider devoted to improving the lives of our patients. The center provides Uber/Lyft to those who don’t drive. Also, they offer exercise, art and other activities for their seniors to promote better health at no cost to the patient.\nRole:\n\n Compensation: Base $260,000 + up to 20% of base salary in bonus’\n Bonus’ based on: 1. Patient satisfaction rating 2. Living the company’s values 3. HEDIS compliance 4. Reducing hospital sick days\n Primary Care – Outpatient\n Value Based Care Model\n Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)\n Average patients per day: 12-15\n Panel size: Max of 650\n Shifts: Monday-Friday 8a-5p\n Call Schedule: Based on number of providers in the market (if 32 physicians in market--call 1:32wks)\n Physicians have their own care team with MA/scheduler/ referral specialist and full support from leadership\n\nBenefits:\n\n Health, Vision, Dental (including family options) & paid parental leave program\n Pre-tax or post-tax Roth contributions available.
