- Conducting thorough patient evaluations, coordinating diagnostic assessments, and creating tailored care plans
- Diagnosing and managing acute, episodic, or chronic conditions in cooperation with physicians or independently based on your credentials
- Providing education to patients and their families about preventive measures, wellness strategies, and disease management
- Prescribing necessary medications and therapies within your scope of practice
- Engaging in follow-up care and chronic disease management initiatives
- Working collaboratively with interdisciplinary teams to ensure integrated, holistic patient care
- Accurately documenting patient interactions in electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Graduation from an accredited Nurse Practitioner program
- Active Nurse Practitioner licensure in the state of Texas
- National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or valid equivalent
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or capacity to acquire them within 90 days)
- Strong clinical judgment combined with an empathetic approach
- Exceptional communication skills with patients, families, and clinical teams
