In this role, you will:

• Evaluate patients in both inpatient and outpatient settings and develop tailored treatment plans for their unique needs.
• Implement physical therapy interventions designed to improve mobility, function, balance, and strength.
• Work closely with an interdisciplinary team, including nurses, physicians, and other rehabilitation professionals, to achieve optimal patient outcomes.
• Educate patients and their families about therapeutic goals, discharge plans, and essential home exercises.
• Accurately document evaluations, treatment processes, and patient progress in line with facility policies and regulatory standards.
• Engage in care rounds and interdisciplinary meetings to facilitate seamless care transitions and collaboration.

Education requirements include a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution. A state licensure as a Physical Therapist, alongside CPR certification, is necessary, with additional specialized certifications being a beneficial asset.
