Registered Nurse (RN) / Licensed Practical Nurse (LPN) | Long Term Care

Role: Registered Nurse or Licensed Practical Nurse providing comprehensive nursing care in a healthcare facility. Responsibilities Include Developing and implementing individualized nursing care plans based on medical histories and assessments. Reviewing physician prescriptions, communicating care plans, and coordinating with healthcare team members. Providing physical, emotional, psychological, and spiritual support to residents, families, and friends. Teaching residents and families about health conditions, medications, and self-care procedures. Monitoring and evaluating resident conditions, adjusting care plans as needed, and documenting care activities. Maintaining a safe, clean environment by following infection control, medication, and safety protocols. Managing resident care equipment, supplies, and participating in admissions, discharges, and transfers. Leading and coordinating workflow among nursing staff and ensuring compliance with legal and organizational standards. Qualifications & Skills Active nursing license in the practice state. Effective communication skills and visual acuity. Preferred 1-3 years of clinical or long-term care experience.
