Your role includes:
- Evaluating patients in both inpatient and outpatient environments and crafting individualized treatment plans to meet their unique needs.
- Administering targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
- Collaborating efficiently with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal outcomes.
- Educating patients and their families on therapeutic goals, discharge planning, and necessary home exercises to ensure smooth transitions.
- Meticulously documenting evaluations, treatments, and progress, following facility and regulatory standards.
- Actively participating in team rounds and interdisciplinary conferences to maintain coordinated, high-quality care.
Education requirements:
- A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
Certifications needed:
- Valid state licensure as a Physical Therapist.
- CPR certification is preferred.
- Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory.
Key skills required:
- Previous experience is highly valued.
- Competence in treating patients with complex medical and mobility concerns.
- Excellent communication and teamwork abilities.
- Proficiency in digital charting and clinical documentation tools is expected.
