SUMMARY: Provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. The qualified therapist assists the physician in evaluating level of function, helps develop and implement the plan of care (revising it asnecessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.

 

Schedule: Monday-Friday, 8:00 AM-4:30 PM, No Weekend, No Holiday, and No on-call requirments

Sign On bonus: up to $15,000

Location: Serving Wayne Seneca Ontario & Yates Counties

 

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

Education: Bachelors, Masters or Doctorate in Physical Therapy required.

Experience: Prefer minimum of one year of experience in Physical Therapy in a hospital setting (not including internship).

Licensure: Current New York State License in Physical Therapy

 

Key Responsiblities

• Provide high-quality, patient-centered physical therapy services in the home setting.
• Develop, implement, and update individualized physical therapy plans of care and discharge plans in collaboration with patients, families, physicians, and the interdisciplinary care team.
• Obtain necessary medical orders and authorizations for physical therapy services.
• Perform comprehensive assessments and regularly evaluate patient progress, adjusting treatment plans as needed.
• Provide rehabilitation interventions including gait training, transfers, ADLs, home exercise programs, mobility training, and use of adaptive equipment.
• Educate patients, families, caregivers, and other healthcare team members on disease management, safety, mobility, body mechanics, and preventive strategies.
• Supervise and collaborate with Physical Therapist Assistants and other members of the care team to ensure safe, effective, and coordinated patient care.
• Participate in interdisciplinary patient conferences and collaborate with nurses, therapists, social workers, physicians, and other providers.
• Maintain accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
• Build positive relationships with patients, families, physicians, referral sources, and community partners through professional, respectful, and responsive communication.
• Identify and connect patients with appropriate community resources and services.
• Support quality improvement initiatives, regulatory requirements, patient satisfaction, and agency performance goals.
• Maintain required productivity standards while using agency resources efficiently.
• Participate in orientation, continuing education, professional development, and other learning opportunities.
• Contribute to a collaborative team environment focused on continuous improvement and exceptional patient care.
• Perform other duties as assigned.

 

Salary Range - $67,700 - $115,045
