The Clinical Pharmacist is responsible for identifying, preventing, and/or resolving medication related problems; for providing authoritative information to other health care professionals, patients, and the public; and for assuring that the informational, clinical, and drug distribution components of pharmacy services function as an integrated system to provide patient centered care to the patients and staff of Northwestern Medicine.

Responsibilities include interpreting medication orders to ensure safety and proper use of medications, monitoring medication therapy for significant drug interactions, evaluating the appropriateness of medications, acting as a consultant in pharmacokinetics and adverse reactions, processing medication orders into the Electronic Health Record, maintaining a comprehensive pharmacy patient profile, overseeing the packaging labeling and distribution of medications, compounding and dispensing pharmaceuticals, verifying medication orders before delivery, reviewing laboratory data for appropriateness of medication use, developing and presenting orientation, education and training programs, educating patients and their families about specific medications, responding to emergency situations to assist with medication management, maintaining the hospital formulary, documenting all clinical activities and interventions performed, communicating professional knowledge effectively in both oral and written forms, participating in quality improvement and medication use review activities, organizing, prioritizing, and planning workload within given time constraints, reviewing restocking automated dispensing cabinets, ensuring compliance with hazardous waste collection and disposal procedures, maintaining proficiency and expertise in area/s of practice through continuous professional development, and all other job duties as assigned.

This position is full-time, 80 hours per pay period, 8-hour rotating shift: Monday through Friday hours start times dependent on area of coverage (7:00 a.m., 7:30 a.m., 8:00 a.m.), every 3rd weekend (Saturday and Sunday) start times 1:30 p.m. or 2:30 p.m., 2 holidays per year Looking for experience in ER coverage, centralized and decentralized operations.

Minimum Requirements: Education: Bachelor's Degree in Pharmacy or a Doctor of Pharmacy Degree Licensure: Active Clinical Pharmacist license in the state of Illinois (IDFPR) Preferred: Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.) Completion of a pharmacy residency program or equivalent hospital pharmacy experience
