Job Title: Licensed Practical Nurse/Registered Nurse

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) and Registered Nurses (RN) to join our team at Harbourwood Post-Acute and Rehab in Clearwater, Florida, United States. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians

• Develop and implement individualized care plans in collaboration with interdisciplinary team members

• Monitor patient vital signs and report any changes or concerns to the nursing staff

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions

• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes

• Collaborate with other healthcare professionals to achieve patient-centered care goals

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Florida

• One (1) year of experience as an LPN or RN in a post-acute care setting preferred

• Strong communication and interpersonal skills

• Ability to work in a fast-paced environment and prioritize tasks effectively

• Compassionate and empathetic approach to patient care
