Admissions Registered Nurse

Blue Ridge Care is seeking a Hospice Admission Nurse - RN to support patients and families entering hospice care. Conduct initial assessments, determine eligibility, and develop individualized care plans with the interdisciplinary team. Full-time, Mon–Fri with 8- or 12-hour shifts and holiday/weekend rotation. Responsibilities include admission documentation, patient education on hospice services, and coordinating with physicians, social workers, and care teams to ensure goals are met and care #J-18808-Ljbffr
