As our Care Coordinator RN you will be a vital part of our Clinical Resource Management team, directly impacting patient care and outcomes. This role focuses on optimizing care coordination, ensuring efficient resource utilization, and fostering seamless patient transitions within our healthcare system. Every day you will engage directly with patients, providing comprehensive care coordination that touches their lives. You'll navigate complex healthcare landscapes, advocating for patient needs, facilitating communication among care providers, and ensuring adherence to care plans. Your work will involve operating equipment that supports patient care and meticulously managing resources to enhance the patient experience and achieve optimal health outcomes.

Job Requirements:

• Graduate of an accredited school of nursing and Minimum two (2) years of acute hospital clinical experience, upon hire or
• Masters Other Case Management or Nursing field. and 1 year of experience, upon hire and
• Registered Nurse: NE, upon hire and
• Basic Life Support - CPR
