Physical Therapist (Sign-On Bonus!)

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living care, focusing on improving mobility, independence, and quality of life for older adults. Key Responsibilities Include Offering one-on-one therapy sessions in private residences and senior communities Performing evaluations, developing individualized treatment plans, and delivering evidence-based care Supporting patient wellness, mobility, and functional independence through therapy Documenting care in electronic medical records in compliance with standards Collaborating with an interdisciplinary team of healthcare professionals Educating patients, caregivers, and staff to promote safety and aging in place Utilizing telehealth and virtual care to enhance access and flexibility Qualifications Include Valid PT license or eligibility in practice state Degree from an accredited Physical Therapy program Proficiency in basic computer and EMR use Current CPR certification Physical demands involve: exerting moderate effort, assisting with patient transfers, repositioning, walking, climbing stairs, and driving to care locations, with appropriate PPE use.
