Santana Hills Healthcare Center is a 167-bed skilled nursing and rehabilitation facility in San Jose, CA , dedicated to providing personalized, compassionate care. We are seeking a Certified Occupational Therapy Assistant (COTA) to join our collaborative rehabilitation team.

Under the supervision of a licensed Occupational Therapist, the COTA will provide individualized therapy to help residents improve their functional abilities, independence, and quality of life. This role works closely with residents, families, and an interdisciplinary care team to support meaningful progress and recovery.

Essential Duties and Responsibilities

• Provide occupational therapy interventions under the direction and supervision of a licensed Occupational Therapist.

• Assist residents with activities of daily living (ADLs), including dressing, grooming, bathing, toileting, and feeding.

• Implement individualized treatment plans established by the Occupational Therapist.

• Provide therapeutic activities to improve strength, coordination, balance, endurance, and functional independence.

• Assist residents with adaptive equipment and strategies to promote independence.

• Monitor resident responses to treatment and communicate progress or concerns to the supervising Occupational Therapist.

• Complete accurate and timely clinical documentation.

• Participate in care planning and collaborate with the interdisciplinary team.

• Educate residents, families, and caregivers on therapeutic strategies and safety.

• Promote resident dignity, safety, independence, and quality of life.

Education & California Certification Requirements

• Graduate from an ACOTE-accredited Occupational Therapy Assistant program .

• Earn the required degree from an accredited OTA program.

• Successfully pass the NBCOT certification examination .

• Hold a current and active California Occupational Therapy Assistant (OTA) license .

• Maintain required California licensure and continuing education.

• Previous Skilled Nursing, post-acute, rehabilitation, or geriatric experience preferred.

• Strong knowledge of occupational therapy principles and treatment techniques.

• Excellent communication and interpersonal skills.

• Compassionate, patient-centered approach to resident care.

• Ability to work effectively under the supervision of an Occupational Therapist.

• Strong organizational, time-management, and documentation skills.

• Ability to collaborate effectively with an interdisciplinary healthcare team.

Physical & Professional Requirements

• Ability to perform essential job functions with or without reasonable accommodation.

• Ability to safely assist residents with functional activities and therapeutic interventions.

• Ability to stand, walk, bend, reach, and assist with resident mobility as required.

• Ability to communicate effectively with residents, families, and healthcare professionals.
