Responsibilities Report to a Clinical Manager and collaborate closely with physicians, advanced practice providers, pharmacists, PT/OT/SLP, social work, case management, and scheduling/triage support. Conduct comprehensive assessments and OASIS documentation. Develop individualized care plans. Perform wound and ostomy care, IV infusions, line and catheter management. Educate patients and caregivers. Ensure safe transitions of care and payer communication. Participate in continuous quality improvement supported by clinical decision tools, optimized EHR builds, and real‑time analytics dashboards.
