Registered Nurse - Diabetes Clinic Coordinator

The Fort Mojave Indian Tribe is currently searching for an RN to provide nursing care for our  Special Diabetes Program grant.  This position will provide nursing expertise and leadership in assessing, planning, implementing, and evluating care provided to individuals at risk for Type 1 and 2 Diabeetes.

This poisition is responsible for daily coordination of outpatient nursing/medical assistant staff. This position is required to assist and relieve all nursing staff within the Health Center.

Duties/responsibilities:

  • Provide quality and professional patient care within the clinic. Document patient care provided in a timely manner, following facility policies.
  • Direct daily supervision of all outpatient nursing/medical assistant staff providing care within the outpatient clinic.
  • Conduct case management services for patients of the Fort Mojave Health Center, including facilitating in-patient referrals to other facilities, handling patient discharge arrangements from other facilities, ordering patient and clinic medical supplies and durable medical equipment, etc.
  • Serve as the primary point of contact to track and manage the care of patients with or at risk for Type 1 and 2 Diabetes Mellitus, utilizing professional judgment and applicable Standards of Care as a basis for management, and performing in accordance with Special Diabetes Program for Indians (SDPI) requirements.
  • Assist with coordinating diabetes care and patient visits with other departments for the prevention of diabetes-related complications, including Optometry, Dental, Physical Therapy, Nutrition and other specialty clinics.
  • Supervise and provide administrative guidance, support and assistance, as needed, with the RN Outreach and Community Health Nurses.
  • Complete, or assist in the completion of, monthly, quarterly, bi-annual and annual reports for Tribal/State/Federal programs, grants or contracts associated with the medical aspects of the health facility (ex. Special Diabetes Program for Indians, Arizona State Immunization Information System, etc).
  • Maintain inventory (ordering, stocking, etc.) of medical supplies needed for the outpatient clinic, including medication and immunization ordering and reconciling.
  • Assist lab staff with testing and specimen collections, as needed.
  • Provide nursing education to staff.
  • Create or update policies to fit any necessary changes of the out-patient clinic.
  • Follow all established facility policy and procedures.
  • Coordinate care of patients via a teamwork approach with health facility departments such as Behavioral Health, Community Health Reps., Pharmacy, Respite, Transportation etc.
