This role offers an excellent opportunity to develop your clinical skills while supporting patients to achieve their rehabilitation goals in their own homes and local community settings. Main duties of the job Provide a high quality Physiotherapy service to patients referred to the Community Therapy team. Hold clinical responsibility for a designated caseload of patients. Undertake a high standard of Physiotherapy assessments of patients with diverse presentations using clinical reasoning skills to provide a diagnosis and deliver individualised treatment programmes. Provide advice and education to optimise health, wellbeing and to promote better self‑management of a condition. Work as an autonomous practitioner within the multidisciplinary team. Ensure timely discharge of patients from the service and refer patients on to other health, social care and voluntary agencies as appropriate. Benefits Eligible for NHS Pension.
