Job SummaryThe role of the Licensed Practical Nurse is to work as a member of the primary health care team in a supportive capacity to the Physician and Nurse Practitioners.Qualifications and RequirementsLicense: Current Florida LPN license and certification to administer medication.Current BLS Certificate from nationally recognized organization that includes hands-on skills verification. Online-only or non-recognized certifications are not accepted.Education: Graduation from an accredited LPN program.Ability to use the nursing process, as evidenced by physical and psychosocial observation skills, interviewing techniques, communication, and interpersonal skills, problem-solving, and clinical judgment. Maintain patient confidentiality. Spanish fluency is desirable, but not essential.Confidentiality:Demonstrates knowledge and full understanding of HIPAA patient privacy rights. Maintains confidentiality of all medical, financial, and other sensitive materials and information in printed, electronic, or verbal form, which may jeopardize the privacy of patients and/or employees. Accesses and uses the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes.Work Environment:The nurse works in a medical clinic environment, which is generally indoors in a climate-controlled environment. The working environment may change somewhat between locations. Nurses will be assigned a home location. However, location assignments will change from time to time, and the nurse must be able to cover at any location, as assigned, either temporarily or on a long-term basis.Essential FunctionsGreets patients and visitors in a friendly, professional mannerUnderstands that our patients are our priority and maintains a respectful, pleasant and polite demeanor at all timesResolves patient issues as appropriate – notifies appropriate supervisor if unresolvedMeasures and documents patient’s primary problems, including vital signs, basic labs, psycho-social history and medications in order to prepare the patient for examination by the provider, anticipating the need for required supplies and equipmentReports to the provider any significant social or other data obtained during conversation with patient or his/her familyManages nurse schedule and sees that patient’s are notified if appointments are cancelled or modified to avoid patient abandonmentPerforms general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards of practice and established scope of practice for licensed practical nurses and as prescribed by physician or nurse practitionerObserves, records, and reports patient’s condition and reaction to drugs and treatments to physiciansAttaches lab and radiology results to patient chart for provider review and notifies provider of test results outside abnormal parametersAssists medical assistants in the completion of medication prior authorizations and sees that they are completed in a timely mannerRecognizes signs/symptoms and/or patient changes and notifies ProviderReviews and upload temperature logs on vaccination refrigerators and freezers to maintain compliance with Florida Shots, identifies and reports any excursions promptlyPlaces orders, monitors inventory for vaccinations, monitors vaccine expirations, vaccine compliance through the Florida Shots online system based on vaccination usage and inventory on handInventories, orders and restocks supplies and vaccines as neededCreates and documents in care plans and self-management support flow sheetsUtilizes nurse schedule to assess patients, educate patients, and administer vaccinations and/or other medications for nurse visitsReturns patient calls/voicemails and places calls as requested by provider in a timely manner. Documents this process in the electronic medical recordProvides patient education to clients utilizing the electronic medical recordWorks as a member of the care team to review medical records for completeness of data prior to the visitTracks, records, and scans radiological procedure orders for providerActively participates in identification and determination of conditions to ensure high quality care to clients/patients and their familiesAttends required meetings and participates in health center work as requestedAdministers immunizations per CDC guidelines and documents administration appropriately in the patient’s medical recordConducts care team huddles daily for efficient communication among the team, assigns back-up in absenceParticipates in Population Health Management activitiesPerforms phlebotomy/lab tests according to lab policy and proceduresMaintains quality and aseptic techniques using standard policies and proceduresPerforms testing/procedures per provider orders such as EKGs, ear irrigation, dressings, ace wraps, nebulizer treatments, and assists Medical Assistants when neededPerforms quality control tests per established protocolWashes and prepares instruments for sterilizationDemonstrates priority setting and organizational skillsExhibits punctuality and dependability in attendance and work habitsPerforms other related job duties as required by the Chief Nursing OfficerHFHC has reviewed this job description to ensure that essential functions and basic duties have been included.
